Families grappling with the challenges of progressive illness often struggle to find clear information, resources, and emotional support. To address this growing need, Hospice of the Chesapeake and Chesapeake Supportive Care are debuting an innovative podcast series titled “Reimagining Hope.” The series, launching on December 4, aims to equip listeners with the tools and knowledge to navigate advanced illness with clarity and confidence.

Hosted by Rachel Jordan, Director of Advocacy & Legislative Affairs for Hospice of the Chesapeake, the podcast will feature expert guests who provide practical insights into managing the complexities of serious illness. Jordan, who is also a nurse practitioner, brings her personal and professional experience to the role, offering relatable, accessible advice for caregivers and patients alike.

Insightful, Practical Conversations

“Every day, I see patients and their families struggling to understand what resources and services are available to them,” said Jordan. “We’re living with serious illness longer than ever before, and each stage—from diagnosis to curative treatment and end-of-life care—brings unique challenges.”

Jordan, who describes herself as a caregiver, Army veteran, and community advocate, says her passion is raising awareness about available care options. “This podcast is about helping families reimagine hope by offering comfort, skills, and a sense of peace in the face of overwhelming circumstances,” she added.

The podcast’s debut episodes address critical issues, including medication management, coping with grief during the holidays, and ensuring comfort through hydration and nutrition at the end of life. These initial topics reflect the wide-ranging struggles that families encounter daily while caring for loved ones.

Episode Highlights

The first three episodes of “Reimagining Hope” will include:

  1. “When is it too much? Managing Mom and Dad’s Medications”
    Guest: Dr. Sonja Richmond, Vice President of Medical Affairs & Hospice Medical Director at Hospice of the Chesapeake.
    This episode explores strategies for managing the often-overwhelming number of medications prescribed to elderly patients.
  2. “Navigating Grief During the Holidays”
    Guest: Jennifer Ward, Bereavement Counselor at Chesapeake Life Center.
    Listeners will gain insights into coping with loss during one of the most emotionally charged times of the year.
  3. “Sustaining Comfort: Hydration and Nutrition at the End of Life”
    Guest: Dr. Sonja Richmond.
    This discussion addresses essential care measures for ensuring comfort and dignity in the final stages of life.

As part of Hospice of the Chesapeake’s commitment to community outreach and education, “Reimagining Hope” is supported by the John and Cathy Belcher Institute, whose contributions make the programming possible. The podcast is designed to provide concise, actionable advice for caregivers, blending expertise with real-world experience.
